• 2 passport sized/style photographs.
• Copy of birth certificate or passport - currently registered JSC members do not need to provide this
• NCYSA Medical Release x2 signed copies (this no longer needs to be notarized)
• Signed 2010-11 JSC Player/Club Expectations Agreement
• Financial Aid request form and paper work
• Extended Payment Plan request form
• U12 players and up with foreign birth certificates will require international clearance. A check for $10 is required payable to USSF. If you are already with JSC and did this previously, there is no need to do it again.

Attendance to your specified registration meeting is important. This will involve completing any outstanding paperwork and may also include attending a team meeting with your Coach and Team Manager.

In the event tryouts are extended due to weather cancelations we have registration make up dates on Monday June 7th and Tuesday June 8th.
