2012-2013 Registration Night Schedule & Information
Required Registration Documents:
- Academy $150 Initial Registration Fee + $115 Uniform Fee
- U11/U12 $250 Initial Registration Fee + $115 Uniform Fee
- Photos will be taken during age group registration (otherwise 2 - passport size headshot current photos on photo paper need to be submitted for player cards. The size should be 1 inch by 1 inch).
- Copy of birth certificate or passport - currently registered JSC members do not need to provide this
- NCYSA Medical Release x2 signed copies (this no longer needs to be notarized)
- Financial Aid request form and paper work
- Extended Payment Plan request form
